You can configure custom questions in the Forms tab to gather essential information from both referrers and referrals, ensuring that the referral process is smooth and effective. This allows for a more personalized approach while maintaining the necessary information integrity.
Navigating to the Form Setup
You can navigate to Referrals > Programs > select the program of your choice > Forms. Then click Edit and then click + Add Custom Question.
Configuring Referral and Referrer Questions
Click on Add Custom Question and choose if you want to create new questions for referral or referrer.
While most questions in the form are predefined and customizable, please note that these predefined questions cannot be deleted.
You can make a question optional by checking the Make it optional box, or you can make it mandatory by unchecking the box.
Similar to the referral questions, referrer questions can be customized or new ones added to capture additional information if needed. You can also create up to 3 custom questions(cumulative of referrer and referral).
After you are done, click Next Step.
Form Preview
As you configure the form, the right-hand side of the screen displays a real-time preview. This allows you to see how the form will appear to users when they fill it out.
The preview accurately reflects the form's layout, including whether questions are mandatory or optional. This ensures that the final form meets the desired specifications.
